Staff Consent Form for Filming & Photography

Please confirm if you do or do not consent to Wenta* using images of you caught in video recordings, and/or photographs, taken/recorded by Wenta.

If you consent to them being used by HR and for other internal Wenta Organisational purposes, as well as marketing and publicity related purposes. This includes their use in other Wenta publications, then you understand that they may be published on social media channels or on the Wenta website, other websites or elsewhere.

I understand that (if my consent is given):

• My images will be held in accordance with the GDPR guidelines (the General Data Protection Regulation);
• The images of myself captured in the video recordings and/or photographs will be the copyright of Wenta and any other intellectual property which arises in the photographs/recordings will also belong to the Wenta;
• I hereby agree to assign all property rights in my photographs and/or recordings to Wenta;
• I hereby agree to waive all rights in my performance in the film and/or recordings to Wenta.
• I can ask Wenta to stop using my images at any time, in which case efforts will be taken to prevent them being used in future digital and offline publications but they may continue to appear in publications already in circulation.

If the recording is going to capture me speaking (e.g. an interview, presenting information, etc), I also agree that I will only include any material in the recording which is the intellectual property (including copyright) of another party, if I have their permission or a licence to do so and irrevocably licence Wenta to use and sub-licence any copyright in the words spoken (once fixed by the recording).

I understand that (if my consent is not given):

• If I have asked Wenta to stop using my images, efforts will be taken to prevent them being used in future digital and offline publications, but they may continue to appear in publications already in circulation.
• Wenta will need to contact me via my email address after receiving the submitted request to decline image rights consent, as we will need to identify who you are to ensure we no longer use your image(s).
• If I want to avoid being in any future Wenta video recordings, and/or photographs then I must make Wenta aware of this as soon as possible to avoid my image being taken. This includes informing Wenta before images are taken at any events, training sessions, and any other Wenta related activities that involve video recordings, and/or photographs being taken.
• Images of me may still be kept for HR purposes and therefore fall under separate GDPR guidelines.
