How to Complete a UK Certificate of Origin

A Certificate of Origin proves where your goods come from — and it’s often essential when exporting. This guide walks you through how to complete each section of the form, so you can export smoothly and confidently.

Box 1 – Consignor
Must reference a UK-based business. If an agent is applying on the exporter’s behalf, include both names.

Box 2 – Consignee
Add the name and address of the overseas recipient. If goods are going elsewhere than the invoice address, show both parties. If delivering to a UK buyer acting on behalf of an overseas client, state “To order for despatch to [destination]”.

Box 3 – Country of Origin

  • Use “United Kingdom” for UK-made goods

  • Do not use “England”, “Wales” etc. alone

  • For non-UK goods, name the country (no group terms like EFTA or EU)

  • For mixed consignments, list each origin separately — alphabetically

Box 4 – Transport Details
State mode of transport: e.g. “Seafreight”, “Airfreight”.

Box 5 – Remarks
Leave blank.

Box 6 – Goods Description
Include marks/numbers, packaging details (e.g. “3 cartons”), trade description, and clear product details. Avoid vague terms. Ensure the goods match your invoice. If space runs out, use more forms or add a “see invoice number...” note. Draw a line under the last item and cancel blank space.

Box 7 – Quantity
State gross and net weights, or relevant measurements.

Box 8 – Official Use Only
Leave this for the issuing body.

Pink Application Copy
Must match the original certificate and include a signature. Kept on file with your invoice for verification.

Box 9 – Agent Declaration (Application Form)
If you’re applying as an agent, declare your name and role here.

What’s not accepted:

  • Corrections using Tippex or erasures

  • Unclear or negative phrases (e.g. “Said to contain”)

  • Boycott statements

  • Unknown origin

Evidence Required (Examples):

  • Your own invoice declaration

  • Supplier/manufacturer invoices

  • Previous Certificate of Origin

  • UK-only signed declarations

Arab-British Certificates – Key Differences:

  • Include manufacturer name in Box 6

  • Use "Printed & Published by..." for printed goods

  • Must state vessel nationality if named

  • Evidence required for foreign goods

  • Originals only for invoices

  • No signatures allowed on Box 6 (except Libya)
